Ramos goes into the examination room with Rachel. Today before receiving an eye donor she should do some of the tests recommended by the doctor.
Ramos performs several examination techniques to ensure the donor's match with the corneal recipient of the eye.
First there is the technique of corneal transplantation can be differentiated into total corneal transplantation (penetration) or partial (lamellar). In corneal penetration transplantation, it will be done replacement of the entire cornea layer, while in lamelar cornea transplantation is only done on a portion of the cornea layer of the damaged patient.
Patient preparation for corneal transplantation consists of informed consent stages, availability of corneal donors, general and eye health examinations, anesthesia and preparation before surgery.
Informed consent is done by giving the patient a brief and clear explanation about the cornea transplant procedure that will be done in the form of an overview of the procedure, goals, risks, and alternative therapies. If you agree to take action, the patient will then be asked to sign the informed consent file.
The most important part of the cornea transplant procedure is the availability of donors from the eye bank or local hospital. A person when agreeing to give a donor cornea, then must do an eye examination, covering the condition of the corneal epithelium whether there is a wound or arcus senilis.
Screening examination of conditions that can be contraindicated to become a donor is the examination of hepatitis B, retinoblastoma, keratitis and endophthalmitis, HIV, and herpes simplex. Once the conditions are met, then the enucleation or retrieval of the cornea will be done a few hours after the death of the donor.
Corneas are restored in culture and antibiotic media, with a temperature of +4β. Shortly before the transplant, the donor cornea is first checked for feasibility, in the form of folds, swelling of the stroma layer, and conditions of endothelial metabolic function of the cornea.
Patient preparation begins with a general examination. Then, an examination to assess the presence or absence of contraindications, such as a very bad visus condition or anatomical abnormalities of the cornea that can affect the success of the procedure.
In addition, examined factors complicating surgery, such as eye conditions in the form of glaucoma and dry eyes, allergic conditions, and metabolic conditions of patients in the form of hypertension, diabetes mellitus, and impaired kidney or liver function. It is important to ask about the medication the patient is currently taking, especially blood-thinning medications such as aspirin, which should be discontinued at least 5 days before the procedure is performed.
On eye examination, anamnesis about complaints experienced today, especially pain and discomfort that can interfere with daily activities. Disturbing pain can be an indication of action even though vision function cannot be achieved optimally. Other anamnesis is a history of eye diseases, such as trauma and previous eye surgery.
Routine examination of the eye in the form of sharpness of the visus, examination of the anterior segment of the eye with a flashlight and slit lamp, followed by examination of the posterior segment with an ophthalmoscope or funduscopy. Visus no light perception is a consideration not to be transplanted, unless the patient has severe pain.
Other tests may be considered depending on the patient's condition. Some of the tests that can be considered to be done are;
Fluorescein test and seidel test
Aberometry
Corneal pachymetry
Keratometer autorefactor or corneal topographer
Optical coherence tomography (OCT)
Other supporting examinations performed are a complete blood examination and urinalysis for the preparation of anesthesia procedures.
Rachel remained faithful to accompany the husband to the examination. He could not wait for Ramos to see as usual.
"Mas, don't be afraid that I'm here" his hand held Ramos' hand tightly.

The two couples came out of the doctor's office. Rachel still remained faithfully suing her husband and helping the man walk. As Rachel said, she'll be Ramos' eyes. He'll be Ramos' foot when Ramos can't see. He'll be Ramos' hand when Ramos can't feel. Rachel will be everything to Ramos.
"Please enter Sir. Miss," Heru opened the car door.
"Thank you Heru" Rachel's smile was so sweet.
"Ck, honey, you must be smiling at Heru, right? Don't smile at him, honey" Ramos protested.
"Why is it, Mas?" Rachel nodded her head inexhaustibly. Her husband is possessive of her.
"I don't like it, honey. You can't smile until I see. Only I can see your smile" said Ramos. He was not willing if many saw Rachel's smile. Moreover, Rachel is getting more beautiful. It could be that the single Heru likes his wife.
"Yes Mas. Ja. Come on in" said Rachel. Understandably if her husband is possessive, men do like it so.
While Heru was instantly clumsy. How dare he argue. Although he was actually a little irritated by Ramos who was possessive of his wife. Heru was not blind who could not see Rachel's smile. Unless she doesn't see Rachel it's natural that she can't see Rachel's smile.
Rachel sat next to her husband. He looked at Ramos with a smile.
"Mas, you'll see soon. Hopefully after this we live happily. With the children" she hugged her husband's arm.
"Amin Darling. I can't wait to see your face" said Ramos. Ramos groped his wife's cheeks. "You're getting prettier, baby" Ramos smiles, his eyes looking straight ahead but his lips smiling happily.
"Where do you know I'm beautiful?" Rachel's temptation half chuckled.
"Your skin is smooth, honey. Just like Gerra's skin. You must be very beautiful" exclaimed Ramos praising his wife. Since the first time she met Rachel, she had indeed admired the beauty of this woman.
"Mas wrong. I'm getting ugly, Mom. Getting old. Especially now that I'm the mother of three children" Rachel said.
"No Honey. You are so beautiful. Even if you have another child, you'll still be beautiful" Ramos said.
Rachel chuckled softly. Ramos looks like he's the one who says he's beautiful.
While Heru only smiled knots while focusing on driving. He who is single can only languish in thinking about the fate between two couples who are in the middle of the kasmaran.
