PRELIMINARIES




BACKGROUND


Elimination is the process of removing the body's metabolic waste either in the form of urine or bowels (feces). Miction is the process of emptying the bladder when the bladder is filled. The body system that plays a role in the process of urine elimination is the kidneys, ureters, bladder, and urethra.


This process occurs from two main steps: The bladder is progressively filled until the tension in its walls increases above the threshold value, what then triggers the second step is a nerve reflex called the micti reflex (micturition reflex) that seeks to empty the bladder or if this fails, at least raises awareness of the desire to urinate. Although the mictional reflex is an autonomic reflex of the spinal cord, it can also be inhibited or induced by the center of the cerebral cortex or brainstem.


The bladder is innervated by the nerves of the sacral nerve (S-2) and (S-3). Sensory nerves from the bladder are sent to the spinal cord (S-2) until (S-4) are passed to the mictional center of the central nervous system. The center sends signals to the bladder to contract. At the time of destrusor contracting internal spinter relaxes and external spinter under ******awareness will play a role, whether to mictiksi or detained.


When abdominal contractions increase bladder muscle contractions, usually no more than 10 ml of urine is left in the bladder called residual urine. Normal urine elimination depends on the individual, usually after work, eating or waking up. Normal daily 5 times.


Defecation is the discharge of feces from the anus and rectum. It is also called the bowel movement. The frequency of defecation in each person varies greatly from several times per day to 2 or 3 times per week. The amount of stool also varies from person to person. When waves of peristalsis push feces into the sigmoid colon and rectum, sensory nerves in the rectum are stimulated and the individual becomes aware of the need for defecation.


Regular elimination of the remnants of intestinal production is important for normal body function. Changes in elimination can cause problems with the gastrointestinal tract and other parts of the body. Because bowel function depends on a balance of several factors, each person's elimination patterns and habits are different.


Clients often seek help from nurses to maintain normal elimination habits. Sickness can avoid them according to a regular program. They become devoid of the physical ability to use the normal toilet facilities of the home environment can present obstacles to clients with changes in mobility, changing needs of bathroom equipment. To deal with client elimination problems, the treatment must understand the normal process of elimination and the factors that affect elimination.

Organs that Play a Role in Urine Elimination


The organs that play a role in the elimination of urine are the kidneys, ureters, bladder and urethra.




Kidneys


The kidney is a retro peritoneal organ that consists of the right and left kidneys of the backbone. Kidneys act as a regulator of the composition and volume of fluid in the body. The kidneys also filter part of the blood to be removed in the form of urine as a waste substance that is not needed by the body. The kidney is made up of nephrons, which are units of kidney structure that number approximately one million nephrons. Through the nephron, urine is channeled into the pelvis of the kidney and then channeled through the ureter to the bladder.




Bladder (bladder, buli-buli)


Is a bag consisting of smooth muscles that serve as a reservoir of urine. In the bladder, there is a layer of muscle tissue that extends in the middle and circular referred to as detrusor and serves to remove urine. Processes of Packing


Bleeding is the process of emptying the urinary vesica. Vesika urinalia can cause nerve stimulation when urinalia contains approximately 250-450 cc (in adults) and 200-250 cc in children.

Urine is removed from the urinary vesica, but is still retained by an external spinter. If time and place allow, it will cause relaxation of the external spinter and urine is likely to be expelled (sad).

FACTOR – FACTORS AFFECTING ALVI ELIMINATION


a) Age


At the age of the infant control defection has not developed, while at the age of the elderly control defecation decreases.


b) Diet


Fibrous food will accelerate the production of feces, the amount of food that enters the body also accelerates the defexation process.


c) Liquid intakes


Less fluid intake will cause the stool to become hard, due to increased fluid absorption.




d) Activity


Tonsus muscles of the abdomen, pelvis, and diaphragm will greatly help the defecation process. Peristaltic movement will make it easier for fecal material to move along the colon.


e) Physiological


The state of anxiety, fear and anger will increase peristaltic causing diarrhea.


f) Position during defecation


The squatting position is a normal position when defecating. Modern toilets are designed to facilitate this position, allowing the individual to sit upright in the front direction, expel intra-abdominal pressure and secrete muscle contractions of – of his thigh muscles (Wartonah , 2004).

Urinary elimination disorder is a condition in which an individual experiences or is at risk for urinary elimination dysfunction. Usually people who experience urinary elimination disorders will be done urinary catheterization, which is the act of inserting a teter collarbone into the bladder through the urethra with the aim of removing urine.
