Before undergoing a bone marrow transplant, people with blood cancer need to undergo a series of tests and procedures to evaluate general health, and ensure a transplant can be done. Depending on some conditions, the evaluation may take several days or more.

The next process is that the surgeon or radiologist will implant a long thin tube (intravenous catheter) into a large vein in the chest or neck. The catheter, often called the central pathway, usually stays in place for as long as treatment lasts. The transplant team will use a central pathway to insert transplanted stem cells, drugs and blood products into the body.

If a transplant using the stem cells themselves (autologous transplant) is planned, people with blood cancer undergo a procedure called apheresis to collect blood stem cells.

Before apheresis, the patient will receive daily injections of growth factors to increase the production of stem cells and move the stem cells into the circulating blood so that they can be collected.

During apheresis, blood is drawn from a vein and circulated through a machine. The machine separates the blood into several parts, including stem cells. These stem cells are collected and frozen for future use in transplants. The remaining blood is returned to the body.

If a person with blood cancer undergoes a transplant using stem cells from a donor (allogeneic transplant), the patient requires a donor. After the donor is found, stem cells are collected from the person for transplantation.

Stem cells can come from blood or bone marrow donors.The medical team of the transplant will decide which is better done based on the situation of the patient.Other types of allogeneic transplant use stem cells from cord blood (transplantation cord blood). 

A bone marrow transplant can be performed after the patient has completed the examination process. On the day of the transplant, stem cells are inserted into the body. The transplant infusion is painless and the patient will be awake during the procedure.

After a bone marrow transplant, new stem cells entering the body will travel through the blood to the bone marrow.In time, these stem cells will multiply and make new, healthy blood cells, it's called engraftment. 

After a bone marrow transplant, the patient will remain under strict medical care. If the patient has an infection or other complications, it is recommended to stay in the hospital for a few days or sometimes longer. 

Depending on the type of transplant and the risk of complications, a person undergoing a bone marrow transplant should remain near the hospital for several weeks to months to allow close monitoring.
